COLLECTION GUIDE
Instructions for Q-tip swab collection for Confidential Paternity Test (2 people):
GENERAL NOTES:
* Wash your hand throughfully with soap prior to collection;
* Read the instructions carefully and make sure that you understand every step.

1. Prepare:
* 2 regular size envelopes (1 per person);
* 6 Q-tip swabs (3 per person);
* 1 shipment envelope. Make sure that this envelope is large enough to include 2 regular size
envelopes and the submission form.
2. Mark the first collection envelope with an ID and its relation to other samples (ex. "SAMPLE A - ALLEGED FATHER").
Note: The ID does not have to be a name - you can choose any identity in order to distinguish the sample, ex. "SAMPLE A")
3. Cut one end of each Q-tip swab
4. Collect the sample as follows:
* Insert remaining end of the Q-tip swab into your mouth;
* Press it against your inner cheek. To ensure that the enough pressure is applied, you should feel the Q-tip on the outer part of the cheek;
* Roll the Q-tip 10 (ten) times back and forth;
* Repeat the same procedure on the other side of your mouth with the same Q-tip swab;
* Place the collected sample into the marked envelope;
* Repeat the process two more times with two remaining Q-tip swabs;
5. Seal the envelope using water.
Note: Please, DO NOT LICK THE ENVELOPE!
6. Follow the same collection steps (2-5) for the next person.
7. Fill-out the submission form or include a note with you contact information and details about the case.
8. Mark the shipment envelope with Forensic Genetic Center's address:

9. Attach a stamp with the proper value to the shipment envelope and mail.
10. In the final shipment envelope please include:
* the collected samples;
* the submission form;
* payment (if the test is not purchased online)

* 2 clear zip lock bags (1 per person);
* 1 shipment envelope.
2. Mark the first zip lock bag with an ID and its relation to other samples (ex., "SAMPLE A - ALLEGED FATHER").
Note: Please, DO NOT LICK THE ENVELOPE!
3. Pluck 5-10 hair strands from the head
4. Make sure that the plucked hair strand contains the root since that is the site of DNA extraction
5. Place the plucked hair into the marked zip lock bag.
6. Follow the same collection steps (2-5) for the next person.
7. Fill-out the submission form or include a note with you contact information and details about the case.
8. Mark the shipment envelope with Forensic Genetics Center's address:
